Neuroleptic Malignant Syndrome in an Atypical Patient - A Diagnostic Dilemma

With the awareness about mental health improving over the last decade, even in developing countries, the diagnosis and management of psychiatric disorders among the elderly has also increased. The common disorders in the elderly population include depression, anxiety disorders and psychosis, and recent literature suggests a higher incidence over last few years. Quetiapine is one of the most commonly used drugs in the geriatric population, mainly because of its low adverse effect profile. In this case report, we discuss a case of Neurpleptic Malignant Syndrome (NMS) in a 69 year old female, manifesting the syndrome at the dose of 200 mg per day of Quetiapine. NMS is uncommon in the geriatric population, with only a few case reports published. Our case is rare in more ways than one, making it difficult to clinch the diagnosis, however responding completely to treatment.
